Course Description

Our host, José Posas, MD, FAAN, is joined by Logan Schneider, MD and Margaret Kay-Stacey, MD. First, we will discuss a case of persistent hypersomnia, despite being on supposedly appropriate therapy. Next we’ll review a case looking looking for the reason for the patient’s “spells”. We will have expert discussion for the last half of the webinar with audience participation and Q&A.

Learning Objectives

As a result of this self-assessment activity, the learner will be able to:

  • Identify the key diagnostic features of narcolepsy and how they can overlap with other sleep conditions, which can often delay the diagnosis of narcolepsy or distort the clinical picture.
  • Understand the work up for a patient with suspected narcolepsy and the limitations of the clinical testing (actigraphy/sleep diary, overnight polysomnogram and multiple sleep latency test). Identify when to consider testing the patient’s CSF for hypocretin/orexin levels.
  • Identify characteristics of cataplexy as differentiated from other types of spells (eg, duration, maintenance of awareness, progressive loss and rapid resumption of muscle tone, preserved eye movements, etc.).
  • Enumerate the pentad of symptoms associated with narcolepsy: cataplexy, excessive daytime sleepiness (EDS), hypnagogic/hypnopompic hallucinations, sleep paralysis, disrupted nocturnal sleep.
